The mental health care system in Victoria, Australia, has been a topic of reform and improvement for decades, yet the progress remains elusive. As a psychiatrist with over 40 years of experience, I find it disheartening to witness the repeated cycles of scandal and false hope. The landmark Royal Commission into Victoria's Mental Health System, which was supposed to be a turning point, has instead become another mirage.
The commission was established due to the public mental health system's collapse, which occurred after the closure of standalone mental hospitals in the 1990s. This led to a downsizing and neglect of the system, with the Victorian government finally admitting to this policy failure in 2018. The commission's expert advisory committee, which I had the honor of chairing, presented a comprehensive blueprint with 74 recommendations for transformation.
Initially, there was unprecedented political will and community support. Bipartisan agreement and a joint sitting of the parliament launched the final report, signaling a potential breakthrough for mental health care. The Victorian government committed to a signature reform, backed by a new levy to secure the necessary funding. The then-deputy premier, James Merlino, took on the role of mental health minister, and regional mental health boards were established to independently commission services.
However, the momentum has since faded. The issue has been marginalized and buried within the Health portfolio, with a revolving door of ministers. The Department of Health, which assumed a more prominent role, made a critical error by entrusting officials lacking mental health expertise and who disregarded expert advice. The mental health levy, which generates over $1 billion annually, is now suspected of being diverted to support the broader hospital system and state finances, causing a breach of trust.
Despite the government's claims of progress, the reality on the ground remains dire. Mental health services have deteriorated, with acute units still being custodial and non-therapeutic. Clinical research, vital for healthcare advancement, is endangered, and recent treatment advances are not accessible to patients. The core system is not a priority, leading to worsening risks for patients, staff, and the community.
This implementation failure is evident, and the government's spin cannot conceal it. The undermining of watchdog and accountability mechanisms, such as the regional boards and the Mental Health Commission, further highlights the bad faith. People with severe mental illnesses continue to be marginalized, suffering in their homes or on the streets.
To break this cycle, government failures must be acknowledged, and a new direction is necessary. As the state election approaches, Victorians deserve transparency about mental health's priority status and the commitment of both political parties to reform. It is time to address the systemic issues and ensure that mental health care receives the attention and resources it desperately needs.
